ANS lists it as a Bologna Papal States coin, which I would consider very surprising if true. I want to add it to Numista, but I highly believe it has to go in a different issuer. But which one?

Have not found any coins from some Constantine looking left that look anything similar though. Maybe a fake/fantasy/medal what not.

Salve, in rete ho visto che non è propriamente una moneta ma una medaglia d' oro coniata a Bologna del valore di 2 Ducati per l'incoronazione di Carlo V del 1530.Quindi penso sia attribuibile al Sacro Romano Impero.

Indeed, it looks like the model for the radiate bust of Charles used on some of his later Low Countries coins:
Agreed the wording is strange. Maybe the meaning is constance (steadiness) rather than something having to do with Constantine. So, “an emperor steadfast in peace?”
Anyway, since it's a medal apparently, I'm not interested in adding it myself. If one of you wants to, you're welcome of course.
