Best option is to test it, if its silver or not , then compare weight and diameter with a real one.
Also looking details for searching the possible errors, like the typography of the lettering , the edge shape and the bust.
One thing suspicious is some granular effect on the legend, which can be left by a mold.

I find it strange that it has no patina. Compare with the excellent example in Numista https://en.numista.com/catalogue/pieces35206.html with the proper portrait.
Yes, you aare right, there is no patina. A coin for more than 100 years old MUST HAVE A STRONG PATINA.
I recognize that a patina is one of the tools I too use to detect a fake. However it is not uncommon for high grade coins with a lot of luster to have been conserved in a way as to limit if not to stop the formation of a patina. Other tools are also abused to stop or erase patinas such as dipping. The result of these are commonly seen in older blast white ms American coins. In this case lack of or existence of a patina is not a do or due argument for or against a coin. I am convinced of its originality due to details on the bust and especially the denticles and edge of the coin. If you need more assurances, wait for other opinions or post it on CCF. They have some very good experts on Spanish colonial pieces.
I collect anything: If it's Italian or Italian states i collect it even more!

But if you got it from your grandfather, It must be a real Reale. I received so many silver from the British India and Europe from my granny and when I took them to the Jewelry shop, they say it is pure silver.
But I need a reale, but I couldn't find from someone else, that is why I will just collect Pestas of the 19th century.

@Nyx I also think that the coin is in a good condition. It has no suspectable edges or rims and patina is common.Reales coins are actually blurred but it is okay. I also recommend that you magnify the coin surface more to find if it has too much porous holes. So, chances are less for fakes, It is a real coin. Wonderful coin actually!
